At one point they see a guy asking for help from a distance, and Joel says it's probably a trap. When questioned, he says something like "Because I used to do that". So him dying is not a big deal, it's just that they went for the "Trevor killing Johnny because he's bored" style where it leaves the player feeling like all the shit they went through in the previous game was for nothing.

Ellie also creates a lot of orphans in the village later on.
